Our At-Home Physical Therapists help patients safely increase strength, range of motion and coordination to improve overall mobility and function.

We help patients prevent or best manage their conditions so that they achieve long-term health benefits. We examine each patient and develop a plan – in coordination with a patient’s physician – using treatment techniques to promote the ability to move, reduce pain, restore function and prevent disability. In addition, we strive to help prevent the loss of mobility before it occurs by developing fitness and wellness programs to help our patients lead healthier and more active lifestyles – and a better overall quality of life.

Occupational Therapy

Occupational Therapy with Heritage OnCare assists seniors through therapeutic services focused on resuming and/or enhancing their ability to perform activities of daily living and to increase independence. Our Occupational Therapists also work with patients and their families or caregivers to promote independence and maintain safety in their homes.

Occupational therapy is like physical therapy, but there are key differences. While physical therapy focuses on the ability to move the body, occupational therapy focuses on a patient’s ability to perform daily living activities. For example, a physical therapist will work with a stroke patient to build back muscle strength, while an occupational therapist will work with a stroke patient to practice basic skills like walking, eating, bathing and dressing.

Our in-home Occupational Therapy services include:

  • Diagnosis education
  • Disability assessment and evaluation
  • Motor coordination
  • Support with activities of daily living
  • Strength training
  • Individualized home exercise program
  • Transfer training
  • Balance training
  • Postural training
  • Home safety assessment and education
  • Fall Prevention
  • Pain management

Some of the conditions that benefit from OT include:

  • Arthritis
  • Chronic pain
  • Stroke
  • Diabetes
  • Spinal cord injury
  • Brain injury
  • Multiple sclerosis
